Regarding All Visit Types

  • Please note that we do not contract with any insurance providers, including Medicare. We will collect payment at the time of service (cash, credit card, check, flexible savings accounts and health savings accounts). If you are interested in knowing your potential level of reimbursement for our services, please call your insurance provider and inquire about their coverage for “out of network physical therapy,” as well as any applicable deductibles.

  • If you would like a super bill to independently submit to your insurance company for potential reimbursement, you can easily access all your itemized receipts in the patient portal (PT Everywhere). If you need or want help, simply ask the front desk and they’d be happy to show you how!

  • Cancellation Fee: There is a $100 fee for appointments cancelled less than 48 hours in advance.

  • No-show Fee: Appointments that are either not cancelled OR cancelled same-day are the full price of the appointment.

Patient Forms

We use PT Everywhere for all of our automation of forms / payments / questionnaires. After making an appointment, these forms will be automatically emailed to you for completion prior to your appointment.

Regarding All Packages

  • Packages do not include Initial Evaluation visits, but if a package is purchased on the date of the Initial Evaluation, we take $50 from the cost of the Initial Evaluation.

  • You can use physical therapy package visits for any physical therapy visit, regardless of diagnosis/reason.

  • Packages expire after one year from date of purchase.

  • Although we do not refund package purchases, we are happy to allow the gifting of unused visits to friends/family for follow-up appointments within the year of expiration.
